to processing your enquiry.

Initial Consultation & Health Assessment
At your Initial Health Assessment we will usually ask about your current personal, social,
medical and mental health circumstances, where appropriate. We may also ask
about your background and family history, as well as the issues that are affecting
you now. We require this information so that I can assess and decide about our
offer of services to you and to manage the service we provide to you.
